Licensing Program Analysts (LPAs) Tyler Reyes and Jonnisha Culbert conducted an unannounced annual/random inspection at the above facility. LPAs met with Comeaux, Gwendolyn Theresa Licensee and provided an Entrance Checklist-Family Child Care Homes LIC 126. LPAs conducted a tour of the facility led by Licensee Comeaux. Facility Postings: LPAs observed the required posting in the kitchen Facility License, PUB 394 Notification of Parents\342\200\231 Rights, LIC 9148 Earthquake Preparedness, LIC 610A Emergency Disaster Plan, and LIC 9040 Children Roster. LPAs observed (9) nine children in care during inspection. Per Licensee there are (12) twelve children currently enrolled. Per Licensee Comeaux, hours of operation are Monday through Saturday 6 AM to 9 PM. The licensee\342\200\231s facility serves children newborn to 12 years. The licensee is approved to care for MAX. CAP (WHEN THERE IS AN ASSISTANT PRESENT): 12 - NO MORE THAN 4 INFANTS. CAP 14 - NO MORE THAN 3 INFANTS. 1 CHILD IN KINDERGARTEN OR ELEMENTARY SCHOOL AND 1 CHILD AT LEAST AGE 6. During the inspection, the following individuals were present in the home: Licensee, staff 1, and staff 2. Any individuals residing in the home have been discussed and noted. LPAs were only able to work on the following domains: Page 1 of 2 Physical Plan t: The following was observed during the tour of the facility: LPA observed in the restroom four (4) cans of cleaning spray Glass Cleaner, Lysol, Bathroom Grime Fighter, and Clipper Blades Disinfectant approx. 2:40PM in restroom 1 cabinet behind the door which poses immediate risk to children in care. Due to time constraints, LPA will return at a later date to complete remaining CARE Tool domains. Exit interview conducted with Licensee Comeaux and a copy of this report was provided. Appeal rights were discussed and distributed. A copy of this licensing report dated 4/3/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Please see attached LIC 809-D.
