What the inspector wrote

On 4/3/2025, Licensing Program Analyst (LPA) Loyce Phillips, conducted a case management inspection to follow up on an Unusual Incident, reported to the Department on 3/21/2025. LPA was greeted by Director, Jacqueline O'Sullivan. LPA toured the facility and took a census of the children. There were LPA observed a total of 237 children in care with 43 staff members. Description of the incident : On 3/20/2025 at 11:36am, Child 1 (C1) was playing on the climbing structure, reaching for a bar when he missed a step and fell on the grass. Staff 1 (S1) was supervising and witness when C1 fell. S1 got an ice pack from the classroom and applied it to his neck. C1 nose was swollen and started to bruise and bleed. S1 took C1 to the health office. The health assistant S2 continued to applied ice and called parent. Parent arrived at 11:43 to pick child up. Parent took child to see his medical provider. Child was given a doctor's note to return to school on 3/21/2025. During this inspection, LPA toured the facility, took photos of the play area and provided best practices regarding the play equipment. Based on the information provided and interviews conducted further investigation is required on this unusual incident. An exit interview was conducted, a conducted, a copy of this report and notice of site visit was provided to Director.
