What the inspector wrote

On 4/15/2025, Licensing Program Analyst (LPA), Loyce Phillips, conducted a visit for the purpose of conducting unannounced Case Management-Deficiencies visit. LPA met with Susana Anton, Program Supervisor as the Director was unavailable today. LPA toured the facility and observed 34 children in care with 6 staff members. Description of the incident : On 3/18/2025 El Segundo Regional Office, Child Care Licensing received a complaint with allegations of staff left day care child locked in a classroom alone. LPA conducted a visit on 3/20/2025 and was made aware that the incident was not reported to the department in a timely manner. Therefore, a Type B citation is issued for reporting requirements. An exit interview was conducted, a copy of this report, appeals rights and notice of site visit was provided to Susana Anton, Program Supervisor.

Supervision shall include visual...This requirement is not met as evidenced by: Based on staff interview statements, staff 1 disclosed she was responsible for leaving child in a locked classroom. C1 was without visual supervision, which poses an immediate health, safety and/or personal rights risk to the children in care.

(1) Events reported...(C)Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not met as evidence by: An unusual incident occurred on 3/14/2025 and the Director did not report the incident to CCLD. This is a potential risk to the health and safety of children in care.
