Correction check·April 28, 2025

On 4/28/2025, Licensing Program Analyst (LPA), Loyce Phillips arrived to the facility to conduct a Plan of Correction visit and met w ith Director, Jacqueline O'Sullivan . There was 231 children present with 46 staff members during today's visit. On 4/15/2025, Licensee was cited a Type A for Neglect and Lack of Supervision. The purpose of today's visit is to ensure the facility has provided the families with the LIC 9224 Acknowledgement of Receipt of Licensing Report. LPA observed a sample of children records and observed completed LIC 9224 Acknowledgement of Receipt of Licensing Report are in the files. Director has implemented and trained staff on new procedures regarding dismissal, head injuries and active supervision. Accordi ng Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and Health and Safety Code, no deficiencies cited. An exit interview was conducted. A copy of this report, notice of site visit, deficiencies clearance letter were discussed and provided to Licensee.
