What the inspector wrote

On 5/13/2025, Licensing Program Analyst (LPA) Loyce Phillips arrived to the facility to conduct a case management follow up inspection. LPA was greeted by Director, Jacqueline O'Sullivan. LPA toured the facility and took a census of the children. Upon arrival there were 262 children present today with 50 staff members. Description of the incident : On 3/4/2025, Child 1 (C1) was playing a chase game and fell. C1 hit his head on the turf in the backyard play area. C1 reported he fell to Staff 1 (S1), and ice was applied. C1 later complained of an earache and S1 called parent to come pick child up. During today's visit LPA interview staff and observed children playing outside and around the play structure, located in the front area. LPA also observed the back play area and provided additional best practices regarding the play equipment. Based on the information obtained, interviews conducted and observations of the play yard area. C1 was playing a chasing game with other children and fell. C1 hit his head on the turf, no other equipment or objects were around child when he fell. Child has not returned to the facility after incident. According to Title 22 and the Health & Safety Code no violations have occurred, and no deficiencies are cited. A notice of site visit was given and posted for 30 days. Exit interview conducted and report was reviewed with Director, Jacqueline O'Sullivan.
