What the inspector wrote

On 8/04/2025 Licensing Program Analysts (LPA)s Judy Laureano and Cristina Castellanos conducted an unannounced case management inspection at the above-mentioned facility for the purpose of ensuring that standards are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes. LPAs were greeted by Director Jacqueline O\342\200\231Sullivan who guided LPAs on a tour. LPAs observed 92 children and 15 staff members providing care and supervision during the start of the inspection. LPAs were following up on an Unusual Incident Report (UIR) made to the El Segundo Child Care Regional Office (ESCCRO). Based on the UIR received 7/07/2025, report states that the child named on report was walking down the stairs, missed a step and hit her chin. Staff 1 (S1) was at the bottom of the steps and took C1 to the health office. Ice pack was applied, and parents were called. Parents took child to urgent care. During today's visit LPAs observed and inspected the area where the incident occurred. Based on the information obtained, interviews conducted and observations, C1 missed a step which caused an injury to C1\342\200\231s chin. Child returned to the facility on 7/8/2025 with no restrictions. Child's chin area needs to stay dry. At this time no further investigation is needed. According to Title 22 and the Health & Safety Code no violations have occurred, and no deficiencies are cited. A notice of site visit was given and posted for 30 days. Exit interview conducted and report was reviewed with Director, Jacqueline O'Sullivan.
