On Monday, July 28, 2025 Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management inspection to follow up in regards a self-reporting incident that occurred on July 16, 2025. On July 16, 2025, Palmdale Regional Office received an unusual incident report stating a child tripped on the sandbox and sustained an injury on the right arm. During this visit, LPA interviewed the child, staff #1 who observed the incident and inspected the outdoor play area. During this inspection, LPA Rivera observed present staff fingerprinted cleared and facility to be within ratio. LPA observed a total of 74 preschool with 10 staff present providing care and supervision. Based on information provided and interviews conducted the incident does not appear to have been the result of any violation of Title 22 regulations, therefore, no deficiencies were cited. The incident was an accident. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report was reviewed with administrator Katri Tyni. A copy of the report was also provided.
