What the inspector wrote

On Monday,May 4, 2026, Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management inspection to follow up in regards a self-reporting incident that occurred on March 3, 2026. On March 3, 2026, Palmdale Regional Office received an unusual incident report stating a child stated teacher pinched the finger when not listening while playing with the purple rice that was in the sensory bin. During this visit, LPA interviewed the child and staff #1 and did not observe purple rice in the sensory bin. During this inspection, LPA Rivera observed present staff fingerprinted cleared and facility to be within ratio. LPA observed a total of 36 preschoolers with staff #1, staff #2, staff #3, staff #4, and staff #5 present providing care and supervision. Based on information provided and interviews conducted the incident does not appear to have been the result of any violation of Title 22 regulations, therefore, no deficiencies were issued. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report was reviewed with Assistant Director Christine Mitchell.. A copy of the report was also provided.
