What the inspector wrote

On 12/05/2024, Licensing Program Analyst (LPA) Lisa Clayton and LPA Brittany Lovest conducted an unannounced Required 3-year Inspection at The First School Pine St Campus. LPA\342\200\231s met with Licensee Kim Israel and Director Satoe Unoki. LPA\342\200\231s observed 29 children being supervised and cared for appropriately by 6 fingerprint cleared staff. Facility hours of operation are Monday \342\200\223 Thursday 8:00am \342\200\223 4:00pm. Fridays 8:am \342\200\223 3pm. The CCC provides snacks and water. LPA Clayton and LPA Lovest toured the facility indoors and outdoors for a Health and Safety Inspection. Capacity and limitations as specified on the license are being maintained. All children are under visual supervision of a teacher at all times. The CCC is in compliance with Title 22 regulations regarding Staff to Student ratios. There are no pools, ponds, Jacuzzis, spas or any other bodies of water on the property. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked cabinet area. LPA\342\200\231s inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. The CCC has front and back play yard. The surface of the outdoor activity spaces and the structures are maintained in a safe condition and are free of hazards. All hand washing facilities and toilets are in safe and sanitary operating condition. LPA\342\200\231s inspected napping equipment placement, bedding and storage for safety. Water temperature, toilet paper, paper towels, safety and sanitation were inspected. Floors in the facility are clean and safe. LPA Clayton and LPA Lovest conducted a Health and Safety inspection of the food storage area and observed it to be free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is to be discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. LPA Lovest reminded Licensee and Director that all food must be labeled and dated after opening. The CCC had the water tested and received testing results on 02/29/2024 \342\200\223 no lead exceedances. Santa Monica Fire Department renewed the CCC fire clearance May 2024 which expires June 2025. The CCC has a switch operated Fire Alarm bell outside of the main room, at the back play yard entrance. LPA\342\200\231s observed a fully charged 3A:40B:C Fire Extinguisher and a working smoke detector in the converted garage/Art Classroom. LPA\342\200\231s observed a working carbon monoxide and 2 working smoke detector in the main classroom, a working smoke detector in the in the block center, and a working carbon monoxide detector, a working smoke detector and a fully charged 2A:10BC Fire Extinguisher in the cubby room. LPA\342\200\231s observed a current disaster drill log on the Parent Board. LPA Lovest inspected a First Aid Kit in the cubby room and reminded licensee and Director to inspect or renew the kits yearly. LPA Clayton reviewed 6 children\342\200\231s files and observed files to have current contact information of parents and authorized representatives, and authorization for medical treatment, signed Parent\342\200\231s Rights and signed Personal Rights receipts. LPA Clayton reviewed 6 employee files and observed CPR/First Aid and Mandated Reporter training certificates as required for opening and closing staff. LPA\342\200\231s reminded Licensee and Director that facility roster is a legal document and to refrain from using white-out or crossing out (drawing a line through) the children\342\200\231s names and ensure that the roster is complete and current. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. Incidental Medical Services \342\200\223 IMS services are currently being provided. LPA Clayton provided Director with the IMS sample plan and instructed her to forward a copy to the office immediately upon completion. LPA Clayton instructed the Licensee and Director to store any unrefrigerated medication in a locked box, inaccessible to children in care. For IMS information see PIN 22-02-CCP . Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. LPA Clayton reminded the staff to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, no Deficiencies were cited, Technical Violations were issued (see LIC 9102). Licensee and Director will ensure that teaching staff take the required 16 hours of Health and Safety training. Licensee will ensure that the staffs MMR vaccine/immunization records are updated and in compliance with immunizations required for child care providers no later than 12/15/2024. Licensee will submit Teacher Qualification forms and transcripts for all teaching staff and a Directors Qualification form and transcripts for Satoe no later than January 1, 2025. During the exit interview the report was reviewed and a copy was provided to Director Satoe. LPA Clayton posted a Notice of Site Visit which must remain posted for 30 days.
