What the inspector wrote

On 05/10/2023 Licensing Program Analyst ( LPA) Doris Whitmore conducted an unannounced Case Management. After conducting a record review facility was unable to show proof that one teacher was missing immunization. Per Title 22, Chapter 6, of the California Code of Regulations, the following deficiency is being cited: ( See next page 809D) Exit interview was conducted and a copy of this report, appeal rights and Notice of Site Visit was provided to the Director Dawn Speciale
