Deficiency is cited in accordance with Title 22 of the California Code of Regulations and/or Health & Safety Codes. An exit interview was conducted, a copy of this report, appeals rights and a notice of site visit were discussed and provided to Director.

This requirement was not met as evidenced by: Based on interview statements from S4 disclosing the facility had a male substitute and he probably did not change the children pull-ups. Also P1, P2 and P5 disclosed they have picked up their child from the facility in a soiled pull-up more than once, which poses a potential health, safety, or personal rights to persons in care.
