What the inspector wrote

On 12/7/2022 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at Dvorskaya Alternative School DBA: West Hollywood College Preparatory School located at 1317 N. Cresent Heights Blvd. Los Angeles, CA 90046 and met with Director Jina Mukhuradze. LPA toured the facility indoors and outdoors and a census was taken; 3 children and one staff member were present during the start of the inspection. Entrance to the facility is located on Fountain Blvd.; the side of the facility. LPA observed the following 3 preschool classrooms. Days and hours of operation are Monday through Friday 7:30 a.m. to 6:00 p.m. Currently facility has 10 children enrolled. There is no swimming pool or other bodies of water on the premises. Medications are made inaccessible and stored in the kitchen top cabinet. Facility does not have any children who are prescribed medication. If medication needs to be refrigerated, medication will be stored in kitchen with all necessary documents. Teachers do the day to day cleaning in the classroom. Outside pest control vendor is contracted to ensure facility stays pest free. Facility is free of flies, insects and rodents. No poisons were observed in today\342\200\231s inspection. Disinfectants, cleaning solutions and other hazardous items are made inaccessible and stored in the staff's bathroom and kitchen cabinet. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. Licensee removed three outdoor bicycles due to missing handlebar covers. They will be inaccessible to children until the equipment is repaired. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Page 2 All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Facility provides snacks and meals. Menu is posted on parent board and emailed to parent. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. LPA observed a water dispenser in the kitchen area where children and teachers are able to access water when needed. When outdoors, teachers take a water pitcher with cups for children to use. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. Facility has a sprinkler system, smoke detector and carbon monoxide detector in each classroom. Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. LPA reviewed Mandated Reporter Training and staff has current proof of completion, Director completed Mandated Reporter Training on 9/26/2022. Director\342\200\231s CPR and First Aid was completed on 1/16/2022. LPA reviewed staff files and 1 out of 2 files were observed to be incomplete. LPA issued a Technical Advisory and director agrees to review files and ensure all necessary documents are completed. LPA reviewed a sample of children\342\200\231s files and observed 2 out of 3 files were incomplete-missing immunization record. LPA provided director with a copy of the LIC 311A to use as a reference to ensure all files have current documents. Page 3 The name of the child care center director or fully-qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. Facility representative, Jina Mukhuradze was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) is NOT being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee facility representative Jina Mukhuradze.
