What the inspector wrote

On 09/04/2025 at 12:31 p.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection due to an incident that occurred on 08/28/2025 and was reported to the Regional Office. LPA met with Monique Randolph Regional Site Director and informed the nature of the visit. Children in care with proper teacher/child ratios observed. There was a total of 27 children staff and 5 staff. According to the UIR RSD (Regional Site Director) said a teacher (T1) witnessed behavioral health aide for disabilities using an inappropriate strategy (C1) and (C2) had altercation and aide instructed (C2) to hit (C1) back) RSD reached out to the Manager of the behavioral Health Company. Investigation is still on going. During today\342\200\231s inspection, LPA conducted interviews with staff during the inspection. Based on the investigation which included interviews further investigation is needed. A copy of this report, Notice of Site Visit, was issued to Regional Site Director Monique Randolph.
