What the inspector wrote

On 09/15/2025, Licensing Program Analyst( LPA) Doris Whitmore conducted a follow up case management inspection regarding an incident that occurred on 08/28/2025. LPA met with Nilda Santiago Site Lead Teacher. LPA observed a total of. 22 children. present and five teachers . On 09/04/2025 interviews were conducted with staff. On 09/15/2025 LPA Whitmore continued with interviews. During staff interview a disclosure was made that the behavioral health aide took the hand of one child to hit another child. LPA Whitmore concluded the case management incident visit. There is one deficiency which is a Type B for Personal Rights. Please see the D- Page. Exit interview was conducted with Site Lead Teacher Nilda Santiago. A notice of site visit was given and posted for 30 days..
