What the inspector wrote

On 10/23/2025 at 9:13 a.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection due to an incident that occurred on 10/13/2025 and was reported to the Regional Office. LPA met with Nilda Santiago- Site Lead Teacher and informed the nature of the visit. LPA toured the facility and observed 28 children and 5 staff. According to the UIR the morning ( S1) observed a bruise on the left side of ( C1) cheek. It appeared old and green in color. When teacher asked child what happened? ( C1) responded in Spanish saying. I was misbehaving and and upset and my mom hit me with the cellphone. Teachers did not recognize the bruise during health check because (C1 was crying) ( S1) called the Child Abuse Hotline and stated it was not reportable. It was determined that it was inappropriate discipline. During this inspection, LPA toured the facility, interview staff and child. LPA obtained a copy of Health Checklist. LPA reviewed ( C1) file and reviewed the Health Check. The Health check is included on the Sign in Sheet. LPA concluded this case management incident visit. No Title 22 violations have occurred, and deficiencies cited. A notice of Site visit was given and appeal rights. Site Visit must be posted for 30 days.
