What the inspector wrote

On 12/10/2025 at 8:40 a.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection due to an Unusual Incident/ Injury Report that was called in on 12/08/2025 and reported to the Regional Office. LPA met with Nilda Santiago, Site Lead Teacher, and informed her about the nature of the visit. At the time of the visit there is a total of 22 children and 5 staff. According to the Unusual Incident/ Injury Report, December 2 nd through December 8 there have been 3 confirmed cases and 3 potential cases reported to the facility. The facility went ahead and sent a notice to parents; director has put in a work order and staff sanitizing high touch surfaces. Daily Health Checks are conducted at the time of arrival. At this time there are are no more cases that are reported on Hands, Foot and Mouth. LPA obtained a copy of the Exposure Notice, Illness Exclusion Form, and Illness Management. Exposure Notices have been given to the parents. All six children have not returned to school. LPA reviewed the policy on Illness Management. LPA concluded this case management visits no Title 22 violations have occurred, and no deficiencies cited. A notice of site visit was given and posted for 30 days. Exit interview conducted and report was reviewed with Nilda Santiago, Site Lead Teacher.
