What the inspector wrote

Licensing Program Analysts (LPA) Brittanee Cleveland conducted an unannounced case management visit on 05/26/2026. LPA arrived to the facility at 09:20AM and met with Cheryl Gartsman, Director, who guided LPA on tour of facility. The purpose of today’s visit is for the continuation of a complaint investigation. At the time of visit, there are 52 Children and 7 Staff. As LPA was interviewing Director, it was discovered that the incident that occurred had not been reported to the department. LPA advised Director that any unusual incident that occurs at the facility must be reported within 24 hours and documented within 7 days. LPA has issued a Type B deficiency due to failure to report this incident in a timely manner. LPA has given Director until May 29, 2026, to complete an Unusual Incident/Injury Report (LIC 624), to clear this deficiency. During this visit LPA conducted staff interviews for the complaint allegations that were reported to the department. Staff 1 and Staff 2 stated that child was observed to be pointing at their ear but did not cry or scream. Parents were contacted to pick up child each time. Staff stated that since the incident occurred child had not been back to school. Staff 2 states that parents stated the child needed a surgical procedure and no medical paperwork was provided to the facility. The child’s return date has not been disclosed. LPA advises the child should be monitored upon returning to facility. Director stated though this may have been a mistake the child will be looked after with care. LPA advises if any unusual incidents or injuries occur at the facility, it shall be reported in a timely manner. --- Page 1 The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted and report was reviewed with the director, Cheryl Gartsman. --- Page 2
