What the inspector wrote

Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection on 06/25/2025 due to a self-reported incident that occurred at the facility. LPA arrived at the facility at 8:30AM and met with Luis Sanchez, Principal, who guided LPA on a tour of the facility. There were 57 children in care and 13 staff present upon arrival. The purpose of the visit was to follow-up on an incident that was reported to the department. The incident that occurred on 06/12/2025, was reported to the Department on 06/13/2025, via email. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Staff #1 and Child #1 were injured when a bell that was used to announce end of outdoor play fell from the anchor the bell was connected to. During an interview with Staff #1, disclosures were made that they were hit on the head when the bell fell from the wall. Staff #1 reported no injuries and stated no medical attention was needed. During an interview with Child #1, disclosures were made that they were hit on the head with the bell fell from the wall. Child #1 disclosed that an ice pack was provided and did not cry. Principal disclosed that Child #1 parents were notified. Parents did not seek medical attention for Child #1. Principal identified the bell that fell on both Staff #1 and Child #1. LPA observed that repairs had been made and bell is secured to the wall.(photo taken) There were no deficiencies cited during today’s inspection. The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted and report was reviewed with Luis Sanchez, Principal.
