What the inspector wrote

On 12/18/25 \302\240Licensing Program Analyst (LPA) Angela Luz, met with Director Ericka Andrews and conducted an unannounced Annual/Random inspection.\302\240 Also present were 7 staff supervising 24 infants. The facility is within capacity and ratio compliance today. LPA provided licensee the Entrance Checklist (LIC 125) and the facility was toured indoors and outdoors for a Health and Safety Inspection. Days and hours of operation are Monday through Friday from 6:30 am until 6:00pm.\302\240 The facility also has a preschool license (197411253).The facility has two classrooms under the infant license: infant room and toddler room (older infants). Both classrooms were inspected for age-appropriate furnishings, equipment, and adequate storage for children\342\200\231s belongings. LPA observed furniture and equipment are in good condition and free of sharp, loose or pointed parts. LPA observed sufficient toys and play materials in good condition. The infant classroom had 4 swings. Staff informed LPA they use the swings to help soothe infants. After 10-15 minutes, depending on how long it takes for the infant to settle, staff take them out. If an infant falls asleep in the swing, staff will transfer the infant to their crib. Forms of discipline used at the facility are talking with the child and giving the child space. LPA observed the napping room in the infant room has a half door, and a half wall with windows on top. LPA observed the narrow walkways between cribs and advised Director on how to adjust the cribs to create a more accessible walkway for staff. Older infants in the toddler room sleep on cots. Bedding is provided by the families and is sent home to be washed weekly. LPA informed Director that infants sheets shall be changed daily or more often if needed. LPA advised to request families bring in enough sheets for the week and sent them all home on the last day of the week. 15 minute sleep checks that document and signs of distress are conducted and documented. Infants sleep with a sleeping outfit that has the arms and legs separated. No infants use sleep sacks. LPA observed changing tables within an arm reach of a sink and meet the safety standards. Facility has separate staff and child designated bathrooms. Children's toilets and faucets are in safe and sanitary operating condition. The children can reach their designated sinks, toilets, and supplies. LPA observed trash cans without lids. Director explained that solid waste diapers are thrown outside into a trash can that has a tight fitting lid. LPA observed floors and surfaces were free of debris and hazards, the presence of a fully functional carbon monoxide detector, smoke detector/fire alarms, and a fully charged fire extinguisher that is accessible throughout the facility. Cleaning solutions are kept in out of children's reach or in the supply closet, inaccessible to children in care. The doorknob to the supply closet has a child lock. No poisons were observed to be accessible to children during the inspection. The center is equipped with a fully stocked first-aid kit that is available in the classrooms. The isolation area for when a child is sick is the office. The facility provides breakfast, lunch, and PM snack. All center provided food items are properly labeled & stored separately from cleaning supplies. Food preparation area is free of debris, hazardous materials, and vermin, and is adequately equipped. Menus are posted at least one week in advance, dated and available for review. Drinking water is accessible by children in the classrooms. LPA observed formula and breastmilk bottles labeled with the child's name. LPA reminded Director that bottles, dishes and containers of food brought by the infant's authorized representative shall be labeled with the infant's name and the current date. The facility has a waiver on file for staggered playground use. The outdoor play area is fully fenced and has shade available. LPA observed one large hole and several small ones in the resilient material in the back of the playground.\302\240 LPA noted that one of the poles that supports the shade structure has exposed foam. Director informed LPA they have already spoken to the owner about remedying the materials. LPAs observed age-appropriate outdoor toys and materials for the children. Drinking water is provided in water bottles or disposable cups when children are outside. Per Director, there are no firearms or other weapons on the premises. There are no bodies of water, or free-standing water accessible to children. All individuals subject to criminal record review have a clearance and have been associated to the facility. LPA reviewed the facility roster & personnel report and copies were obtained. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Director was reminded to ensure staff renew Mandated Reporter for Child Care Providers and Pediatric CPR/First Aid every two years. LPA observed all required postings in a publicly accessible area. LPA reviewed the sign in/out sheets and noted they were in compliance. Fire/disaster drills are recorded and performed at least every six months, last drill was on 10/16/25. LPA reviewed 5 random children files and 6 random staff files during today's inspection. Infants aged 0-12 months old files contained the Individual Infant Sleep Plan (LIC 9227) signed and dated by the infant's authorized representative. 2 of 5 child files reviewed did not have an Infant Needs and Services Plan (INSP). 5 of 5 files reviewed did not have a current INSP. Director stated staff are informed of any changes. LPA reminded Director that Infant Needs and Services Plan shall be updated quarterly or as often as necessary to be current for all infants 0-24 months old. 4 of 6 staff files were complete. 2 of 6 staff files were missing Health Screening Report (LIC 503). 1 staff file was missing a current mandated reporter certification. LPA advised Director to use the Entrance Checklist (LIC 125) to audit staff files to ensure all are complete. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16 . For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 ( PIN 21-21.1-CCP .). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP . LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES This facility provides Incidental Medical Services \342\200\223 IMS.
