What the inspector wrote

Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection on 09/20/2024 due to self reported incidences that occurred at the facility. LPA arrived at the facility at 1:30PM and met with Carmen Vasquez , Principal. There were 49 children and 18 staff present upon arrival. The incident that occurred on 08/12/2024 was reported to the Department on 08/14/2024, via telephone. The facility did not reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that interactions between Staff #1 and Staff #2 may or may not have violated the personal rights of children in care. LPA obtained a copy of the facility roster and other pertinent information regarding this incident. LPA was not able to conduct interviews with Staff #1 and Staff #2. Staff #1 and Staff #2 were not present during the inspection. Additional information is needed to determined if the personal rights of children in care were violated. The incident that occurred on 08/14/2024 was reported to the Department on 08/14/2024, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. Information reported to the Department indicated that Staff #3 may or may not have violated the personal rights of Child #1. LPA interviewed Principal and Staff #3 during today's inspection. Based on the information LPA obtained from the interviews, it was determined that the personal rights of Child #1 were not violated. Resources regarding reporting requirements and personal rights of children in care were provided to the Principal. ---Page 1 of 2 There were no deficiencies cited during today\342\200\231s inspection. The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted and report was reviewed with Carmen Vasquez, Principal.
