During interviews Parent 1-3 disclosed that their child wear diapers and their child did not have a diaper rash. Based on observations, interviews, and supportive records information was not revealed throughout the investigation to approve or disapprove a violation occurred. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the above allegations did or did not occur, therefore the allegations are unsubstantiated. No citations given during today\342\200\231s visit. This report reviewed with licensee and copy given. Notice of site visit given and must be posted for 30 days. Exit interview conducted.
