What the inspector wrote

On September 8, 2025, Licensing Program Analyst (LPA) Calloway made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office on March 18, 2025. LPA toured the facility with the representative and observed nineteen (19) children and six staff in active care. All adults 18 years and over have Criminal Record Clearance (DOJ) and Child Abuse Central Index Clearance and are associated with the facility. The reported information involved Staff observing Child 1 inside the classroom with three staff present and during clean up time, C1 fell back in a chair and sustained an injury to the back of the head. LPA conducted interviews with all relevant parties involved. LPA obtained a copy of the incident reports and completed a safety inspection of the classroom. Based on interviews and record review, it was determined that C1 was observed by the staff sitting in a chair. C1 pushed themselves back into the chair using the table causing the chair to tip backwards. C1 fell backwards in the chair hitting their head on the hinge of a play toy and sustained an injury that was bleeding to the back of their head. The staff was in the area and observed the incident, but it happened quickly and there was no time for the staff to react. The staff notified C1\342\200\231s parents and Community Care Licensing (CCL) and the Director conducted interviews with their staff when they returned. The staff took appropriate action. There is no deficiency cited during this inspection. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit was provided to Edna Rodriguez, Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
