What the inspector wrote

On 02/25/2026 LPA Clayton conducted an unannounced Case Management inspection at A Kids Place. LPA was greeted by Teacher Rosibel Adino. LPA Clayton toured the facility inside and outside for Health & Safety inspection and observed 7 children being supervised and cared for by 2 fingerprint cleared staff. On 12/02/2025, during Required 3-year inspection, LPA Clayton reminded Director Lizet of the importance of completing the Blue Immunization cards (CDPH 286) and provided her with the Blue Cards via USPS (mailed on 12/11/2025). LPA Clayton also advised Director Lizet to contact the CA Department of Public Health and request that they mail the Blue Cards to the campus. During today\342\200\231s inspection, LPA Clayton reviewed 7 children\342\200\231s files observed that all files were missing the required CDPH PUB 286 form, which poses a Health and Safety risk to children in care. Based LPA Clayton\342\200\231s file review and observations 1 (one) Type B violation is cited today (see LIC 809D). An exit interview was conducted, and this report and Appeal Rights were discussed and provided to teacher Rosibel Adino. LPA Clayton posted a notice of site visit which must remain posted for 30 days.
