Correction check·March 10, 2026

On 3/10/2026, Licensing Program Analyst (LPA) Lisa Clayton arrived at A Kid\342\200\231s Place Day Care Center to conduct an unannounced Plan of Correction visit. LPA Clayton was greeted by Director Lizet Alvarez. LPA Clayton observed 8 children in care children being supervised and cared for appropriately by Director and 3 fingerprint cleared teachers. On 12/02/2025, LPA Clayton issued the following citations: Type B Citation for: 101227(a)(6) Food service - there were no menus posted for authorized representatives review. On 02/25/2026, LPA Clayton issued the following citation: Type B Citation for: 1596.8595(d)(1)(2) - Posting licensing report by child care facility or home; duration of posting; civil penalty for failure to comply; reports to be provided to parents or guardian of each child receiving services - no signed Acknowledgement of Receipt of Licensing Reports forms in the children\342\200\231s files Type B Citation for: 101220.1(g)(1) \342\200\223 Immunizations - children\342\200\231s files were missing the required CDPH PUB 286 (blue card) The Plan of Corrections for the citations listed above has been met as evidenced by: LPA Clayton observed menus posted on the Parent board and on the refrigerator, both areas accessible for review by the children's authorized representatives. LPA Clayton reviewed the children\342\200\231s files and observed completed California Pre-K and School Immunization Record (blue cards). LPA Clayton reviewed all of the children\342\200\231s files and observed the signed Acknowledgement of Receipt of Licensing Report (LIC 9224) staff reviewed the Food Service Requirements for Child Care Centers video, and LPA Clayton reviewed the written Declaration of Understandings Citations issued on 12/02/2025 and 02/25/2026 have been cleared. An exit interview was conducted. A copy of this report, notice of site visit, deficiencies clearance letters were discussed and provided to Director Lizet Alvarez. LPA Clayton posted a Notice of Site Visit , which is to remain posted for 30 days.
