On Wednesday, February 4, 2026, Licensing Program Analyst (LPA) Hanna Cha met with Director Corina Sifuentes. The purpose of the inspection was to conduct an unannounced case management inspection for an Unusual Incident Report (UIR) received at Palmdale Regional office on 01/27/2026 regarding Child #1 (C1) bumping head on apparatus bar and throwing up. LPA Cha disclosed the purpose of the inspection to the Director. Upon arrival, there were five teachers providing care and supervision for 40 children. LPA acquired a copy of the roster prior to the visit. During the visit, LPA Cha made observations, interviewed C1, and interviewed staff. Based on LPA interviews, no concerns indicated from C1 or staff regarding incident. Staff interviews disclosed consistent information about the incident and prevention measures in place to prevent a similar incident. LPA determined no further follow-up needed. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted and the report was reviewed with Director Corina Sifuentes.
