What the inspector wrote

Licensing Program Analyst (LPA) Saul Valenzuela conducted an unannounced Annual Inspection. LPA met with Principal, Melissa Thomas, who guided LPA on a tour of the facility indoors and outdoors. This is an LAUSD state preschool program located in RM #2 on the premises of Stonehurst Elementary School. The program consists of one (1) classroom; 2. Facility operation hours are Monday through Friday from 7:55 AM to 2:25 PM. This facility has a capacity of 24 children; ages 2 to 5 years old. Upon arrival, the following staff were present during this inspection: RM 2: Staff #1 and #2 with 15 children. Teacher-child ratio are met. All individuals present have obtained a criminal record clearance or criminal record exemption. All staff have received an active criminal record clearance as a condition of their employment with the Los Angeles Unified School District. All children were observed to be under visual supervision of a teacher at all times. LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. Currently, the nurse's office is used as an isolation area. Parents are contacted immediately when children are determined to be ill, and staff are ensuring that children with obvious symptoms of illness are not being accepted. --------------------------------------------------------- pg. 1 of 5---------------------------------------------------------- The facility provides breakfast, lunch, and snack. Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Per Principal meals are dropped of daily by school's cafeteria. All food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. There is drinking water available in the classroom via individual water bottles and water fountains- which are taken outside for children to drink. LPA observed trash cans used for solid waste do have a tight-fitting lid. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Per Principal that there are no poisons stored at the facility. LPA observed that there are smoke and carbon monoxide detectors in the facility. The last Earthquake Drill was conducted on 2/27/2025 . LPA observed sleeping arrangements in form of cots. Per Principal sheets are washed every week by an outsource company. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. --------------------------------------------------------- pg. 2 of 5---------------------------------------------------------- LPA did not review complete staff files during the inspection due to the staff files being stored at the administrative office. LPA will review staff files at a later time and date. If staff files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files. At 10:30 a.m., LPA observed that facility did not report the change of Principal and principal's file was not complete. Children\342\200\231s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov . MyChildCarePlan.org \342\200\223 Centers and Family Child Care Homes facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
