What the inspector wrote

On 2/14/2023, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced case management visit for the purpose of investigating the Unusual Incident Report that was self reported via telephone on 1/18/2023. LPA Suzette met with Lead Teacher Gloria Duban who assisted LPA with indoors and outdoors facility tour. Days and hours of operation are Monday through Friday 7a to 5:30p. LPA observed all required postings in a prominent area. A review of the sign in/out sheet was conducted to verify the current census of children. LPA observed 69 children and 25 adults. All children are under supervision, including visual supervision, of a teacher at all times. Capacity and limitations as specified on the license are being maintained. A walk through of the classroom area was conducted. LPA observed 7 licensed preschool classrooms on the premises. All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection: RM 1: 7 children with a ratio of 3 teachers RM 2 : 6 children with a ratio of 3 teachers RM 3: 12 children with a ratio of 3 teachers RM 4 : 10 children with a ratio of 3 teachers RM 5: 6 children with a ratio of 3 teachers RM 6 : 14 children with a ratio of 6 teachers RM 7: 10 children with a ratio of 2 teachers LPA then met with Principal Desilisa Mitchell who assisted LPA with obtaining the following documents: -Children's Roster (licensing form) . -Copy of SCAR . -Children's Sign In/Out Sheets for the month of November -Daily Routine Schedule for all Classrooms -Staff Work Schedule . -Verification of November Classroom 2 staff Exit interview was conducted. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
