What the inspector wrote

On 12/10/2025, Licensing Program Analyst (LPA) Tyra Chavies conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 10/20/2025. Upon arrival, LPA met with principle, Desilisa Mitchell. LPA informed the principle\302\240 about the purpose of the visit and toured the facility. LPA observed 126\302\240 children being supervised by\302\240 23 staff members. Incident Details: "On 10/17/2025 and employee went in the teachers cabinet where her belongings were stored and found her belongings \342\200\234disheveled\342\200\235. She checked her belongings and the caddy where she kept her belongings\302\240 and noticed that a lidocaine patch missing was missing." On 12/10/2025 LPA Chavies conducted in person interviews with Principle On 11/19/2025 LPA Chavies conducted telephone interview with S#3 On 11/21/2025 LPA Chavies received via email facility roster, personnel documents and employee Handbook On 11/19/2025 LPA Chavies toured the facility indoors and outdoors, observed children in care, and conducted in person interviews with, S#1 and S#2 Based on observation, interviews conducted and review on documentation, the facility followed Title 22 California Code of Regulations and the Health and Safety Codes governing California. No deficiencies were cited. Exit interview was conducted and a copy of the report was provided to principle. Notice of site visit was given and must remain posted for 30 days.
