What the inspector wrote

On 03/06/26, at 11:12 AM, Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced Plan of Correction (POC) Visit. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee Reena Nanayakkara, who guided the LPA on a tour of the facility. There were 8 day care children (4 infants) present during today\342\200\231s inspection. LPA also observed 1 additional staff present at the time of this inspection. During the annual visit conducted on 02/17/26, the following deficiencies were issued: Type A \342\200\223 102416.5(d)(1): Ratio and Capacity. Type B \342\200\223 102425(c): Infant Safe Sleep. Type B \342\200\223 102425(j)(2)(D)(c): Infant Safe Sleep Documentation. For deficiency 102416.5(d)(1), on 02/18/26 the licensee un-enrolled two infants. LPA received copies of the letters provided to the parents. During today's inspection, LPA observed the facility was operating within the capacity and limitations outlined in the license. LPA also observed LIC 9224 Acknowledgement Receipt of Licensing Reports. For deficiency 102425(c), on 02/27/26 LPA received copies of LIC 9227 for the four infants enrolled. For deficiency 102425(j)(2)(D)(c), on 02/27/26 LPA received copies of the 15-minute safe sleep documentation for the infants enrolled. LPA also observed during today\342\200\231s inspection that the sleep forms dated 03/02\342\200\22303/05/26 were complete. The deficiencies noted above have been cleared, and no further action is needed.
