What the inspector wrote

On 6/10/25 at 8:05am, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Case Management-Deficiencies visit. The purpose of the visit is to cite for a deficiency discovered during an investigation. Upon arrival, LPA was greeted and let into the facility by Teacher Lesley Mezacardenes and was advised to speak with Lead Teacher Roxenna Mendez-Pineda for she was in charge. During the tour, xx Preschool children and xx Preschool staff were observed. Assistant Director Amy Perez arrived at 8:30am. toddler RM 7 children with 2 teaches RM 3 14 children with 3 teachers The Center is being cited for not being able to meet the individual needs of a child with disabilities. The Center had two children enrolled with special needs. Five years ago, Child #1 was enrolled in the Center. Approximately two years ago, Child #1 began attending a Los Angeles Unified School District (LAUSD) public school and had been taking a bus from the school to the Center. Prior to Child #1 being enrolled in the public school, a verbal agreement was made between Licensee/Director Lara and the parent of Child #1 that Center staff would collect Child #1 from the bus every day and that Child #1 would continue to receive care and supervision from the Center until Child #1 was picked up. Child #2 was enrolled for approximately 10 months and attended a LAUSD public school from the onset. In April, suddenly and without explanation, Licensee/Director gave both parents notice (informing one verbally and one in writing) that she is refusing to provide the service of retrieving the children from the bus and that they would have two weeks to find child care elsewhere. Per Licensee/Director, it was because Parent #1 informed her that the bus driver complained that it was taking the Center too long to collect the child from the bus and if she was going to discontinue the service for one parent, she had to discontinue it for both. Type B citation is being issued. Exit interview conducted and report was reviewed with Licensee/Director Assistant Director Amy Perez. A notice of site visit was given and must remain posted for 30 days.
