What the inspector wrote
On March 4, 2026, Licensing Program Analyst (LPA) Calloway made an unannounced inspection to the above facility for the purpose of conducting an Annual Random inspection. LPA met with Rubi Trejo, site director, who granted access as the representative. LPA and representative toured the facility inside and outside. LPA observed six infants and eight walking infants in active play and five staff. The facility is open from 6:30 am to 6:30 pm. Monday-Friday. All employees, 18 years and over have a Criminal Record Clearance (DOJ/FBI) and Child Abuse Central Index Clearance and are associated with the facility.
Physical Plant:
The parent wall was in a prominent location near the front entrance of the facility with the required postings. There are two separate clean and safe classrooms, the telephone service, heating, ventilation, and lighting are adequate. Inside the classrooms the daily schedule and activities were posted. There are cubbies for children's belongings that are labeled with their names and photos. The furniture, books, equipment, toys, and materials were aged appropriately and in good condition. There were activity rugs inside the room for play. Representative stated the rugs are cleaned every three months. LPA arrived in the infant room and observed water overflowed from the sink onto the floor and carpet. Representative stated something clogged the drain and caused water to run onto the floor in the diaper changing area and the carpet near the entrance door. Representative stated they will have the maintenance person dry the carpet to prevent mold and placed a temporary rug over the wet carpet area during the inspection.
LPA inspected the sink and it is operable. The drinking water is available inside the classrooms in the form of filtered water pitcher and disposable or personal water cups. Representative stated the water cups are cleaned daily. The napping equipment are cribs and cots and there was enough for all children. Representative stated the bedding is stored separately and washed daily. LPA observed the diapers and wipes labeled and the bottles labeled in the refrigerator. There was a changing table in each classroom with a sink nearby that was operable and with soap and toilet paper available. First Aid Kits were in each classroom in a red backpack along with the Emergency Survival Kit both are in the same back pack in the classrooms and observed sealed. Trash cans have secure lids. The temperature inside the classroom was comfortable. Representative stated the fire extinguishers, smoke detectors, sprinkler system, and carbon monoxide detectors throughout the day care center are all operable and were checked during the last fire inspection. The staff restrooms are in the hallway outside of the classroom. There is a fully equipped kitchen with refrigerator, freezer, microwave, dishwasher, and sink. The kitchen was clean and there were no accessible hazards observed. The facility provides breakfast, lunch, and afternoon snacks. The menus are posted inside and outside the class. There is a confidential allergy list posted on the wall inside the kitchen. LPA observed an appropriate amount of food and snacks. LPA observed 1% nonfat milk and whole milk. The representative stated the reason for having the whole milk is for infants/toddlers transitioning from formula under 2 years old. The staff\342\200\231s food items were in the staff lounge. The chemicals and cleaning supplies were locked in the cabinet under the sink.
Outside:
Outdoor play equipment was inspected for safety, cushioning material, good repair and is age appropriate. LPA observed a medium sized play structure securely anchored in the ground with a canopy for shade and a rubber mat as a cushioned surface. There is concrete and grass for active play. The playground was observed to be free of debris. Drinking water is available in the form of a filtered water pitcher and disposable or personal water cups. Representative stated there are no bodies of water on the premises. LPA did not observe any. LPA observed small plastic play equipment with a small plastic play house. The fencing is wrought iron, observed locked and is secure around the entire playground.
Other Review:
Children and staff files were reviewed. Two staff were missing immunization information. Current Pediatric CPR/First Aid (expires: 6/1/26). Representative stated transportation is not provided. Disaster Drills: Fire Drill was conducted: 2/23/26 and the Earthquake drill was conducted: 2/10/26.
Representative stated the last facility fire inspection was conducted: 8/26/2025. The child/parent roster was complete with all required information. The staff/ child ratios were observed to be compliant; the sign in/out sheets were digital on and APP called LCW. Representative stated the children are inspected for illnesses as they arrive. The isolation area for ill children is in the site supervisor's office until the parent can pick up the child. Representative stated there is no medication being provided to any children. LPA discussed LIC 125 form with the representative.

. to obtain information about the most recent regulatory changes and the Quarterly Updates. All representatives are responsible for knowing the regulations when providing care. All adults 18 years and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers that were constructed before January 1, 2010, to test their water (used for
drinking and food for lead contamination before January 1, 2023, and then every 5 years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at:

sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. State law prohibits baby walkers, bouncy seats, exersaucers, bassinettes or any other items that fall into that category. All infants are sleeping on their backs unless specified with LIC 9227 form and no blankets, toys, or pacifiers attached. Conduct 15-minute nap monitoring and a napping log is kept daily for each infant.
Facility representative was informed of the MyChildCarePlan.org website, a
consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Exit interview was conducted and a copy of this report was read, a Notice of Site Visit (LIC 9213), this report, and Appeal Rights were provided to Rubi Trejo, Director. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
