What the inspector wrote

On June 3, 2025, Licensing Program Analyst (LPA), Calloway conducted an unannounced case management inspection to the above facility. The purpose of the inspection was to follow up on Plan of Correction (POC) for a deficiency that was cited. LPA met with the facility representative who granted access. LPA toured the facility with the representative and observed zero school aged day care children and zero staff in care. All adults residing in the facility have a Criminal Record Clearance (DOJ/FBI) and Child Abuse Index Clearance and are associated to the facility. On April 23, 2025, LPA observed two areas of the carpet in the school aged classroom that had large tears and cited a deficiency. On April 24, 2025, the facility representative sent an email with photos and a work order number that the carpet areas were glued down temporarily and per the representative, a work order was submitted to the facility manager for the carpet to replaced. LPA arrived at the facility and observed at 9:35 AM., one area of the carpet was glued down and not lifting, however, there the other area of the carpet was not repaired and has a long, vertical tear going across the carpet as a hazard. Per representative, the area is high traffic, and the kids play there a lot. There is a Failure to Correct Civil Penalty assessed during this inspection. See LIC 421FC page attached to this report. For $100 per day for 18 days from 5/17/25 POC date. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Rubi Trejo, Facility Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
