Licensing Program Analyst (LPA) Thompson-Miller met with Director, Celina Diaz for a Case Management - Incident inspection involving an Incident Report dated September 22, 2021. The incident occurred on September 17, 2021. Present are 70 children and 10 Teachers. Description of the incident: At approximately 4:42pm Staff #2 was preparing children to go outside, children lined up then went outside, door closed by Staff #2 and began to take a head count by calling each name. Staff #2 then realized Child #1 was not with her, immediately opened the door and Child #1 was approximately 3 feet from the door, starring at Staff #2 when door opened, she was not crying. Child #1 was fine while outside playing and the remainder of the day. Interviews were conducted with parents and staff. Based on information provided and interviews conducted the incident does not appear to have been the result of any violation of the Title 22 regulation, therefore, no deficiencies were cited. Notice of Site Visit was provided. An exit interview was conducted, a copy of this report was read and provided to the Director, Celina Diaz on this date.
