What the inspector wrote

On June 4, 2025, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted on to the Palmdale Regional Office on May 23, 2025. LPA toured the facility with the representative and observed fifty-three-day care children and six staff. The reported information involved Staff observing a Child 1 (C1) trying to climb a chain link fence and was poked on the palm and was given a band aid. C1 told parent staff poked them with a pencil. LPA conducted interviews with the relevant parties. LPA reviewed files and completed a safety inspection of the playground. LPA observed the area of the playground fence that C1 climbed. Per representative, the fence is three and \302\275 feet in height, LPA observed the chain-linked fence that was in good repair, steady and secure during the inspection. Based on interviews and record review, it was determined that the incident occurred around 11:15 AM, the Staff were gathering the children and transitioning from the playground into the classroom for lunch. C1 and another child ran in the opposite direction of the rest of the class and started to climb the fence. Staff gave directives for the children to come off the fence. Once inside the classroom C1 stated they received an injury on their hand while playing on the fence. C1 showed their hand to the Staff, and Staff rendered aid with a band aid. Based on observation, the playground was secure and locked, and the area of the fence shown did not have any visible hazards. Per the facility representative, the camera surveillance did not capture the incident with C1. Staff rendered aid to C1 but did not notify the parent of the incident when it occurred. Licensing was notified after the parent notified the Director. It was determined that the day care center followed the required protocol of rendering aid to C1, but did not follow the proper protocol for reporting the incident to the parents when the injury occurred. There are no deficiencies cited during this inspection. There is a Technical Violation issued during this inspection for Health-Related Services. See TV page attached to this report. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Celina Diaz, Facility Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
