What the inspector wrote

On 01/19/2024 Licensing Program Analyst (LPA) Andrew Alemoh conducted an unannounced Annual Required Inspection at the facility. LPA met with school director Celina Diaz and toured the facility indoors and outdoors. Days and hours of operation are Monday-Friday 6:30 AM to 6:30PM. Infants are using 2 classrooms. Infant Classroom: Age appropriate toys and play equipment were observed. Their are cubbies for the infants to place belongings. 8 cribs were observed and readily available for infants to nap. 15 minute sleep check charts were observed for the infants. A clean and orderly changing table w/ extra diapers and cleaning wipes is readily available. Cleaning supplies is stored in the top cabinet making it inaccessible to children. Two sinks were observed in the classroom, by the changing table and the fridge. The fridge contained bottles readily available for the infants. Walkers Classroom: Safe and age appropriate toys and play equipment were observed in the classroom. Cubbies for children to place their belongings. A clean and orderly changing table with extra diapers and cleaning wipes readily available. Two sinks are in the classroom by the changing table and by the fridge. Fridge contained sippy cups for the children. Cleaning supplies located by the changing table was observed to be locked and made inaccessible to the children. There is a gated play area in the classroom for the children to play. 15 minute sleep check chart was observed in the classroom area. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, and other hazardous items are made inaccessible to children. There are no medications on the premises . Furniture and equipment are in good condition, free of sharp, loose or pointed parts. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The outdoor play area is surrounded by an iron fence.The surface of the outdoor activity space such as the turf grass and rubber matting is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, have cushioning material to absorb falls. There is a kitchen on site, food is prepared, and the kitchen is kept clean and orderly. Breakfast, lunch and snacks are provided to the children. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Facility representative was reminded, Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance, transfer or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. Last fire/diaster drill conducted was on 01/12/2024 . Director are currently certified in Pediatric CPR/First Aid which expires on 03/2024. Mandated Reporter expires 12/24 . All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children’s files and observed files were complete LPA reviewed a sample of staff files and observed files were complete. Menus are posted at least one week in advance where an authorized representative can view them. Per Director Celina Diaz children with Incidental Medical Services are taken to the elementary school nurse (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
