What the inspector wrote

On June 12, 2025, Licensing Program Analyst (LPA) Calloway and Rodriguez, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted on to the Palmdale Regional Office on May 29, 2025. LPAs toured the facility with the representative and observed eleven infants and three staff in active care. The reported information involved Staff separated from infant Child 1 (C1). C1 was left unattended, alone inside the classroom while napping. LPA conducted interviews with the relevant parties involved. LPA reviewed C1’s file and completed a safety inspection of the classroom. Based on interviews and record review, it was determined that C1 was left unattended napping in the classroom for an undetermined amount of time. The staff conducted a meeting with a parent, then walked out of the classroom and to the front office and left C1 alone in the classroom. Staff wrote an incident report, notified C1’s parents, their district manager, and Community Care Licensing (CCL) of the incident. Based on observation of the classroom, the area C1 was napping was near the iron fencing that separates the room the cabinets inside the classroom are secure and locked, there were no visible cleaning supplies or hazards. C1 is a walking infant, the doors were observed locked and secure and there is no way C1 was able to leave the premises. The staff took appropriate action in reporting the incident, however, the care and supervision violation did occur. There is one Type B deficiency cited during this inspection for Responsibility for Providing Care and Supervision for Infants. See the LIC 809D page attached to this report. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Celina Diaz, Representative at the facility. A Notice of Site Visit (LIC 9213) must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
