What the inspector wrote

On June 12, 2025, Licensing Program Analyst (LPA), Calloway and Rodriguez conducted an unannounced case management inspection to the above facility. The purpose of the inspection was for citing deficiencies observed during the inspection. LPA met with the facility representative who granted access. LPA toured the facility with the representative and observed eleven day care infants and three staff in care. All adults working in the facility have a Criminal Record Clearance (DOJ/FBI) and Child Abuse Index Clearance and are associated to the facility. During the walk through, at 10:25 AM, LPAs observed a napping infant (C1) in the infant room that was sleeping on their side. LPAs viewed the infant’s file and there was no LIC 9227 form inside the file during the inspection that indicated the infant had the ability to roll from back to side. Therefore, the Safe Sleep Regulations were not being followed. At 10:55 PM., LPAs observed a facility maintenance person on the infant play yard spraying a chemical on the fence and the infants were outside transitioning into the classroom. Per the maintenance person, they had a work order to spray the fencing for rust. Per the representative, they did not know the representative was spraying the chemicals during the children’s play time. There is a Type B deficiency cited for Responsibility for Providing Care and Supervision for Infants and a Technical Violation for Buildings and Grounds during this inspection. See LIC 809D page and TV page attached to this report. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Celina Diaz, Facility Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
