What the inspector wrote

On 12/15/2025, Licensing Program Analyst( LPA) Doris Whitmore conducted a case management inspection to follow up on an Unususal Incident, reported to the department. LPA Whitmore met with Education Coordinator, Elvira Frescas and toured the facility, LPA observed 33 children present with 9 staff. According to the UIR on 12/5 at 2: 15pm during dismissal time, Parent entered the headstart 01 room where ( S1) was standing holding the hand of ( C1)b as 10 children were seated at the table having snack at the table. Parent asked if the child had done anything wrong.( S1) explained to the parent she was holding the child's hand because he was climbing on the tables. Parent yelled. she grabbed ( C1) by the hand and went through the children's restroom. Parent continued to yell before taking the child home as other parents waited in line for their children. During the inspection LPA toured the facility and interviewed staff. Based on interviews further investigation is needed. A notice of site visit were given and posted for 30 days. Exit interview was conducted and report was reviewed with Elvira Frescas.
