What the inspector wrote

On 12/5/2022, Licensing Program Analyst (LPA) Lillian Casillas conducted an unannounced annual inspection. LPA met with Director, Rivka Rabinowitz, and Assistant Director, Chana Tenenbaum, who guided LPA on a tour of the facility. Day and hours of operation are Monday through Thursday, 8:30AM to 5:30PM, and Friday from 8:30am to 4:30pm. The facility has 3 classrooms: RM 1: Blue (3 - 4 year olds), RM 2: Green (18 months - 2 year olds), and RM 3: Orange (4 - 5 year olds). LPA observed 33 children in care and 8 teacher present during this inspection. Director also teaches. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. LPA reviewed the sign in/out sheet to verify the census. The following ratios were observed: RM 1: 9 preschool children with 3 teachers RM 2: 11 toddlers with 3 teachers RM 3: 13 preschool children with 2 teachers A walk-through of the classrooms was conducted, and classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. LPA observed classroom space to include dramatic play, arts/crafts, reading, individual cubbies for storage and learning activities. The director's office is the designated Isolation area. Director stated children bring their own lunch, 1 snack, and water. Children have the option to enroll in a hot lunch program and facility provides snacks 3x/day. Facility provides filtered water for children to refill water bottles. [CONTINUE ON PAGE 2] PAGE 2 Room 1 and Room 3 shared a restroom with 3 toilets and 3 sinks. LPA observed a changing table for toddlers in this restroom. There are sufficient toilets and sinks to accommodate the facility\342\200\231s capacity. Bathrooms were found to be clean. LPA observed one large l-shaped outdoor play area equipped with age-appropriate play equipment that is in safe condition. The surface of the outdoor activity space is maintained in safe condition and is free of hazards. LPA 2A10:BC fire extinguishers (serviced 5/2022) in each classroom. LPA observed carbon monoxide detectors in each classroom. Each classroom includes 2 children's sinks and 1 adult sink. Facility was observed to be operating within the conditions, limitations, and capacity specified on the license. LPA observed a sample of 3 staff records; all contained documentation of education credits. All staff are trained in CPR and Pediatric First Aid (expiration date 2/2024) and Mandated Reporter training expires on 11/2023. LPA reviewed a sample of 5 children\342\200\231s files and observed files were complete. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Incidental Medical Services (IMS) are currently being provided. Director is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services.
