What the inspector wrote

On 5/6/2026 @ 9:00am, Licensing Program Analyst (LPA) Devon Carus conducted a Case Management Unusual Incident inspection to follow up on the self- reported incident that occurred on 04/10/2026 at Discovery World Pre School located at 550 DEEP VALLEY DR. SPACE #217 Palos Verdes, CA 90274. The El Segundo Regional Office received the incident report on 4/17/2026. Upon arrival, LPA observed 28 children being supervised by 8 staff members. All center staff that was present during today’s inspection had fingerprint clearance and are associated to the designated license number. LPA interviewed the following: Staff member #1 (S1) Staff Member #2 (S2), the Director and the child’s mother Adult #1 (A1) as witnesses. The incident happened towards the beginning of the day during morning indoor play time at approximately 10am and C1 was noticeably upset prior to arrival. S1 initially was holding the child’s hand and the child decided to fall to the floor voluntarily. Instead of letting C1 fall, S1 continued to hold the child’s hand to prevent them from hurting themselves. C1 continued through the rest of the day but was observed to still be acting abnormally by center staff throughout the day. Around nap time, the child was given a health check by staff and was not observed to have any ailments. Additionally, C1 did not notify center staff of any pains or injuries. Staff at the school let C1’s parents know at pick up that she had been acting unusually all day. On this date, the parents of C1 observed that their child’s elbow was dislocated and attempted to re-set the elbow themselves. After this, C1’s parents then brought their child to the doctor where medical staff again re-set the elbow. After this, X-Rays were taken and the child was diagnosed with Nursemaid’s Elbow. Page 1 According to interviews regarding the incident, and narratives provided by S1, S2, the Director and A1, the child’s X-Rays did come back depicting there was a small fracture on the inside of the elbow. According to interviews, and information from the doctor consulting C1’s injury, fractures in the inner elbow generally take place from relocations of joints, and that the attempted re-sets of this elbow may have likely caused the fracture. Additionally, it was observed that the child’s elbow was re-set on two occasions after the child had been picked up from the center prior to their X-Rays being taken. LPA Carus inspected the play area involved in the incident, and observed it to be safe, stable, secure, appropriate for the ages of children enrolled at the pre school, and free of tripping or slipping hazards. Additionally, LPA Carus observed that the incident did not occur due to a lack of supervision by center staff. At this time, based on the available information, no violations will be cited today. The content of this report was read and discussed in detail with Liana Austin, preschool director. An exit interview was conducted. The notice of site visit must be posted for 30 days upon receipt.
