What the inspector wrote

On 6/3/2026 @ 9:00am, Licensing Program Analyst (LPA) Devon Carus conducted a Case Management Unusual Incident inspection to follow up on the self- reported incident that occurred on 05/19/2026 at Discovery World Pre School located at 550 DEEP VALLEY DR. SPACE #217 Palos Verdes, CA 90274. The El Segundo Regional Office received the incident report on 5/19/2026. Upon arrival, LPA Carus observed 29 children being supervised by 7 staff members. All center staff that was present during today’s inspection had fingerprint clearance and are associated to the designated license number. LPA interviewed the following: Staff member #1 (S1) and Staff Member #2 (S2), as witnesses, and Child #1 (C1). The incident happened towards the beginning of the day during morning outdoor play time at approximately 11:20 am and C1 was running outside in the infant/ toddler yard. S2 was watching the children outside and asked them not to run to close to each other. Additionally, S2 was standing at the bottom of the ramp near where the children were running. C1 was running with their friends and tripped on the turf area of the playground and hit their head on the handrail next to the ramp to the classrooms. C1 was examined by S1 and S2 regarding the injury who treated their wound with soap and water, and notified C1’s parent. C1’s parent arrived within the hour and took the child to the hospital. C1 returned the next day on 5/20/2026 with stitches on the wound, and no limitations by the parent or doctor. Page 1 LPA Carus inspected the outdoor ramp, turf area, and handrail of the playground area involved in the incident, and observed it to be safe, appropriate for the ages of children enrolled at the pre school, and free of tripping or slipping hazards. Additionally, LPA Carus observed that the incident did not occur due to a lack of supervision by center staff. At this time, based on the available information, no violations will be cited. The content of this report was read and discussed in detail with Liana Austin, preschool director. An exit interview was conducted. The notice of site visit must be posted for 30 days upon receipt.
