Inspector’s narrative

What the inspector wrote

On 5/8/2025 at 08:15 am, Licensing Program Analyst (LPA) Jeanine Lipsey arrived to conduct a Unannounced Annual Random Inspection visit and was met by Lead Teach Sy Hearn who led LPA on a tour of the facility. An entrance check list was provided. There were 37 children present, being supervised by 10 staff. Director Marine Karagulyan arrived at 9:11am to assist with the tour. At 9:16am Licensing Program Manager (LPM) Emiko Bell arrived to assist LPA. LPA & LPM completed the tour, conducted a staff and children's file review, and completed the care tool. Due to time constraints, the report will be completed at a later date. Based on LPA's observations, the following deficiencies listed on the attached LIC809D (deficiency page) are being cited in accordance with Title 22, Division 12, Chapter 3, of the California Code of Regulations. Deficiencies that are being cited need to be cleared to protect the children's health & safety. Exit interview conducted and report was reviewed with Director Marine Karagulyan . A notice of site visit was given and advised that it must remain posted for 30 days.

    Based on observation, the licensee did not comply with the section cited above in that LPA observed staff changing a childs diaper, the sink was behind the staff and they were not able to reach which poses/posed a potential health, safety or personal rights risk to persons in care.
