What the inspector wrote

On 5/15/25, at 10:05 AM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Case Management visit to clear the deficiencies. Upon arrival, LPA disclosed the purpose of the inspection and met with Director Elizabeth Flores, who guided the LPA on a tour of the facility. There were 35 children present during today’s inspection being supervised by 10 staff. Census: Room#9 Toddlers 18 months to 3 years: 6 children with 2 teachers Room #4: Toddlers 18 months to 3 years 6 children with 3 teachers Room #1 room 3 to 5 years. 12 children with 3 teachers Horizon #2 room 3 to 5 years 11 children 2 teachers During the Case Management-Incident conducted on 2/7/25, the following deficiencies were issued: 101238(a) Reporting Requirements Upon the occurrence, during the operation of the child care…(1) Events reported shall include the following: (C) Any unusual incident or child absence CLEARED 101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.... CLEARED 101170 Criminal Record Clearance (e) All individuals subject to a criminal record review ...(1) Obtain a California clearance or.... CLEARED Page 1 101161 Limitations on Capacity (a) A licensee shall not operate a child care center beyond the conditions.... CLEARED The deficiencies noted above have all been cleared prior to POC date and no further action is needed. Exit interview conducted and report was reviewed with Director Elizabeth Flores. A notice of site visit was given and must remain posted for 30 days. page 2
