What the inspector wrote

On 9/15/25, at 11:55 AM Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Case Management incident visit, Upon arrival, LPA disclosed the purpose of the and visit with Director Elizabeth Flores, who guided the LPA on a tour of the facility. There were 41 children present being supervised by 9 staff. The purpose of the visit was to follow up on an incident reported to the Department that occurred on 8/21/25 via telephone. The facility reported the Unusual Incident to the Department on 08/25/25 which was within the required 24 hours of occurrence. Information provided to the Department indicated a child was rocking back in their chair, lost balance and hit their head on a shelf. LPA arrived at nap time children were sleeping. Census: Room#9 Toddlers 18 months to 3 years: 6 children with 2 teachers Room #4: Toddlers 18 months to 3 years 7 children with 2 teachers Room #1 room 3 to 5 years. 13 children with 2 teachers Room #2 room 3 to 5 years 15 children 3 teachers During the inspection, LPA collected copies of Ouch reports, Email to CCRC staff of New addition to expectation review with children, Children's roster, CCRC Medical Referral, Head injury information given to parents, LPA made observation and took photos of the chair & book shelf, and took photo of the incident reported to the parent on the Learning Genie App. interviewed 4 staff and 1 child. On 8/21/25 after 2:30 pm during snack time Child #1 fell and hit their head. All staff that witness the incident, stated that the child was told by staff #2 to stop rocking or they were going to fall. Staff #2 stated they were sitting a table behind the child and watched the child rocking in the chair. The child was told twice and then after the second time the child fell backward and bumped their head on the book shelf that was directly behind the child. The staff checked the child for any visible injures and did not see or feel any. Staff gave child an ice pack and watched the child for any subsequent issues. Per staff the parent was called between 2:30- 3pm and notified on the Learning Genie app. The parent returned the call and was told the child was doing fine and a picture was sent to the parent on app, of the child doing a puzzle. At pickup the child's guardian received a Head Injury information to look for when the child gets home and a medical referral in case they follow up with the doctor. The child was taken to the doctor as a follow up and per the staff, the child was fine and able to return back to school. The child disclosed they were rocking in the chair and fell and hit their head. LPA observed the chair the child was sitting to be in good working conditions. LPA and Director discussed as a standard of the transitioning to go outside, they normally review expectation with the children. Some of the expectations include: using the play equipment properly, no running in the tricycle area, to share, put materials away, and now they will include, to sit with their feet on the floor. This will be implemented immediately. Exit interview conducted and report was reviewed with Director Elizabeth Flores. A Notice of Site Visit was given and must remain posted for 30 days. The department finds there was not a deficiency in compliance.
