What the inspector wrote

On 10/07/25, at 11 AM Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Case Management incident visit, Upon arrival, LPA disclosed the purpose of the and visit with Director Elizabeth Flores, who guided the LPA on a tour of the facility. There were 40 children present being supervised by 12 staff. The purpose of the visit was to follow up on an incident reported to the Department that occurred on 9/22/25 via telephone. The facility reported the Unusual Incident to the Department on 09/23/25 which was within the required 24 hours of occurrence. Information provided to the Department indicated a window was broken. Census: Room#9 Toddlers 18 months to 3 years: 6 children with 3 teachers Room #4: Toddlers 18 months to 3 years 7 children with 2 teachers Room #1 room 3 to 5 years. 12 children with 4 teachers Room #2 room 3 to 5 years 15 children 3 teachers During the inspection, LPA collected copies police officers card, inspected the kitchen, took photos and interviewed staff. On 9/21/25 there was a break in into the kitchen. Per the churches video, the kitchen is the only place the person went into. The person came in through a window by the back door. The person went into the church and the school's refrigerator and seemed to touch everything. Per staff, they threw everything away that was left in the fridge. On 9/22/25 a temporary fridge was brought in for the children's meals and was placed in the office. The children was still able to eat and play outside but staff made sure they stayed away from the area with the broken glass. The staff blocked off the kitchen with the brown barrier and staff was asked not to go inside. The police came on 9/23/25 and a report was taken. The broken window caused small pieces of glass to go onto the play area. The window was boarded up to cover the extra glass that was still present. The play area was blocked off the the children's furniture, and a brown barrier fence to prevent the children from going into that area. The staff cleaned the dramatic play area, table toys/art area, the water play area, and threw away the green turf and the plants to make sure all the glass was removed. On 9/23/25 the glass was cleaned up and on 9/29/25, during the children's nap time, the window was replaced. The children were able to resume playing in that area again. The department finds there was not a deficiency in compliance. Exit interview conducted and report was reviewed with Director Elizabeth Flores. A Notice of Site Visit was given and must remain posted for 30 days.
