What the inspector wrote

On 10/07/25, at 11 AM Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Case Management incident visit, Upon arrival, LPA disclosed the purpose of the and visit with Director Elizabeth Flores, who guided the LPA on a tour of the facility. There were 40 children present being supervised by 12 staff. The purpose of the visit was to follow up on an incident reported to the Department that occurred on 9/22/25 via telephone. The facility reported the Unusual Incident to the Department on 09/24/25 which was within the required 24 hours of occurrence. Information provided to the Department indicated a child was accounted for. Census: Room#9 Toddlers 18 months to 3 years: 6 children with 3 teachers Room #4: Toddlers 18 months to 3 years 7 children with 2 teachers Room #1 room 3 to 5 years. 12 children with 4 teachers Room #2 room 3 to 5 years 15 children 3 teachers During the inspection, LPA observed the play area, took photos, interviewed staff and collected copies of the refresher training the staff attended, the quiz, the email to staff, the CCRC's care and supervision check list, and the self reflection tool. On 9/22/25 around 10:30 am, during playtime transitioning, Child#1 (C1) was found unsupervised in restroom. There were seven children total and three staff transitioning outside after snack time. Per Staff #3 (S3), they were asked by Staff #4 (S4) to change another child's diaper. S3 proceeded to leave the area while C1 was still in the dramatic play area. S3 asked if they were okay to go and thought staff #2 (S2) was watching C1. Per S2, they were busy bringing out the water, the emergency bag and the ratio chart from the class when they the scanned the play yard and noticed a child was missing. S2 went to the classroom and asked S3 was the child with them and did not see C1 so they immediately went to the restroom, near the dramatic play area, and found the child happily playing with their toys on the floor. The dramatic play area is xx from the restroom. Staff #4 was also present in the play yard, filling the water table but did notice the child was missing until the staff asked for C1 where about. All staff disclosed the child was missing for no more than two minutes. The child was found unsupervised. On 9/23/25 the the lead teacher sent out a memo and discussed supervision of the children with the teachers of room 4. Per the director, on 9/29/25, room 4 teachers, received a refresher training on active supervision protocols. They reviewed the active strategy of care and supervision, each staff filled out a self reflection tool, and they took an active supervision quiz. On 10/2/25, during the October team meeting with all the staff, care and supervision, which included 6 active strategies, were discussed, role playing and a post quiz was included. The CCRC weekly care and supervision check list was also discussed with the staff. Exit interview conducted and report was reviewed with Director Elizabeth Flores. A Notice of Site Visit was given and must remain posted for 30 days.
