What the inspector wrote

On 4/22/26, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced,  Case management incident inspection.  The incident was reported to Community Care Licensing on 04/10/26 via phone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident which occurred on 04/9/26 was that Child #1 C1 injured their head on a table during nap time. Upon arrival, LPA was guided on a tour of the facility by Lead Teacher Sy Hearn , to whom the purpose of the inspection was announced. Census:  There were twenty eight children being supervised by eleven staff. Room#9 Toddlers 18 months to 3 years: 6 children with 3 teachers Room #4: Toddlers 18 months to 3 years 3 children with 3 teachers Room #1 room 3 to 5 years. 11 children with 3 teachers Room #2 room 3 to 5 years 8 children 3 teachers During today's visit, LPA conducted interviews with 2 staff and 1 child, took photos of the area in question, photos of the child's injury from the Learning Genie App and LPA observed the child's nap area. Page 1 of 2 Per Staff #1 (S1) around 12:45pm, nap time, Child #1 (C1) was playing on the cot. Instead of lying on the cot, C1 dove for the cot and hit their head on the near by table. S1 immediately went to the child and assessed the injury and saw a dark bruise forming on their forehead and put ice it. The table is children size, low to the ground with rounded edges. Per S1 C1 never sleeps and plays instead of resting. Staff constantly asked C1 to read a book like the rest of the children that do not sleep, but C1 gets bored and moves constantly. Per S1 children are not forced to sleep but we asked them to stay on their cots to rest. Per C1, they do not like to sleep and hit their head on the table while playing by themselves. The Child confirmed that the teacher came over right way to give an ice pack. S1 stated they called the parent and sent photos of the injury to the parent via the Learning Genie. The child did not show any discomfort through out the day and stayed until their normal pick up time. Child was taken to the doctor and was diagnosed with a small hematoma to the forehead and was released to return to school with no restrictions. LPA observed the area where the child's cot was located. The child's cot was placed in between two tables with small chairs with little room for movement. Staff #2 agreed to move the child to a more open space on the carpet to prevent future injuries and asked the child if it was okay. The child agreed. The department finds there was not a deficiency in compliance. Exit interview conducted and report was reviewed with Lead Teacher Sy Hearn.  A Notice of Site Visit was given and must remain posted for 30 days. page 2 of 2
