What the inspector wrote

On 9/25/2025 Licensing Program Analysts (LPAs) Tyler Reyes and Javier Duran arrived at the above licensed facility to conduct a POC inspection to follow up on the citations that were cited on 09/23/25. Upon arrival, LPAs met with Licensee Gutierrez, Ericka and observed (3) children in care. LPA reviewed the following : 9/23/25 Staff Files: LPA reviewed Licensee and S1 file for the following documentation records: LIC 9052 Employee Rights, LIC 9108 Statement Acknowledging Requirements to Report Child Abuse, Proof of Immunizations, and Mandated Reporting Training Certificate. (1) of (2) staff had required documentation. S1 was missing proof of immunization for measles and pertussis which poses a potential health and safety risk. 9/25/25 Staff Files: Upon record review LPAs reviewed proof of immunization of S1's Flu, MMR, and Tdap. No deficiencies were observed during today's visit. Exit interview was conducted and a copy of the report was given to Licensee with appeal rights. A clearance letter of the deficiency was given and a notice of site visit must remain posted for 30 days.
