What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced Case Management Visit to this facility. While conducting an alternate visit at Shepherd of The Valley Lutheran Church on 4/9/25, the following deficiencies were observed. At 3:30 PM, LPAs observed during file review 3 out of 9 staff are missing proof of current Mandated Reporter Training Certificate. LPA also observed 4 out of 9 staff are missing all the required immunization's. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted with Director Season Eng and Appeals Rights provided.
