What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced case management inspection due to an incident that occurred on 4/21/2026. LPA arrived at the facility and met with Director, Maria Goldman who guided LPA on a tour of the facility. There were 37 preschoolers and 5 toddlers being supervised by 6 staff upon arrival. LPA reminded the Director to call the Department within 24 hours to report the incident and to mail or email the LIC 624 Unusual Incident Report form within 7 days. Description of the incident: Child 1 (C1) was climbing up the play structure when they missed a step and hit their chin. Ice was applied to C1’s chin, the parents were notified, and C1 was taken to the doctor. C1 received three stitches to their chin. LPA interviewed Staff 1 (S1) and Staff 2 (S2). S1 was closest to C1 and immediately provided aid. S1 witnessed the incident but was unable to reach C1 in time to prevent the fall. S2 was present on the playground but did not witness the incident occur. LPA interviewed C1, who stated they were climbing up the rainbow ladder, missed a step, and fell. C1 stated that the teachers came to help them. LPA observed the play structure. The structure appeared large, LPA requested that the facility provide documentation showing that the structure is age-appropriate. No deficiencies were cited during today’s visit. A Notice of Site Visit was issued and must remain posted for 30 days. An exit interview was conducted and a copy of the report was provided to Director, Maria Goldman.
